INTERNATIONAL VISA SERVICE


Use this procedure if:

  • Your current passport is valid for 10 years
  • You are in possession of original passport
  • Your name changed AND your passport was issued LESS than 1 year ago (if passport was issued more than 1 year ago, follow Passport Renewal procedure)



STEP 1. Gather the following documents:


  • Original expired/expiring passport issued within the last 15 years

  • Two passport size photos 2"X 2"  (must be taken within last 2-3 months), cannot wear glasses

  • One Application DS-5504 (click here to download)

  • IVS Order Form (click here to download)

  • ​Certified copy of the marriage certificate, divorce decree or a court order (this document will be returned to you upon passport completion)



​STEP 2: E-mail documents for review (DO NOT SKIP THIS STEP):

Once we receive copies of all of your paperwork, our passport specialist will prepare final application forms for you and will e-mail them to you for review and signatures. Photos can be taken and e-mailed for digital processing. Please contact us for more details.
